To apply to the school, follow these steps:

Step 1:   Links to forms are located at the top of this page
 

________	Completed Application Forms (Student Information Form, Tuition Computation Form, and Enrollment Packet) This should be completed by the parent or guardian.
________	Submit Enrollment Fees (Non-refundable per student)  In the result that admission is not granted, 90% of fee will be refunded
________	Copy of Student’s Immunization Records  An application remains incomplete without submission of the most recent immunization record.
________	Copy of Student's most recent Achievement Test Scores  The most recent achievement test must be submitted, even if test is more than one year old.
________	Copy of student's most recent Report Card Lititz Christian requires copies of student report cards for the current school year and the previous year.  These can be quarter by quarter grades, or semester report cards.
________	Parent Statement of Agreement Please sign the parent agreement found in the Parent-Student Handbook and return to administration.
________	Financial Statement of Agreement Sign and return statement to administration.

Step 2: 
        _______ Interview with school principal.  (Interview will be initiated by the school principal.)

Step 3: 
        _______ Acceptance letter and appointment with Financial manager.  (Appointment will be initiated by the Financial manager.)

Step 4: 
        _______ DIBELS ~ Grade 1-2 
        _______ ACT 89 Math Assessment ~ Grade 1-5 
        _______ 4 Sight Test
